Aligned Care & Connect Hub (ACCH) brings together professionals, businesses and organisations across health, care, wellbeing, ageing, palliative and end-of-life support — and the many services that help people navigate life's transitions.

Because no single person or service has all the answers. But together, we can make it easier to find them.

More than a professional network

ACCH is an online and in-person space to build relationships across all professionals who have conversations with those who are navigating aging, life-limiting conditions, palliative care and dying for themselves or another.

Dissolve boundaries and open bridges between industry sectors, tighten the net, discover complementary services, exchange knowledge, and explore opportunities to work together.

It's about getting to know the people behind the services — understanding what they do, who they support, and how we might help one another to reduce clients slipping through the cracks.

Through meaningful connections, we can:

Connect At Meet and Greets and Online

Connection helps us learn who, what and where beyond our professional circles, discovering we can connect with

People, services and resources, information, tools, events and new ideas we may not know exist, or we know of, but need an introduction to reach out to them.

Within ACC, we aim to support and create space for these relationships, building awareness, understanding, and pathways that benefit both professionals and the communities we serve.

Collaborate on purpose driven activities

Collaboration brings different knowledge, experience and perspectives together to explore ideas, share resources and identify opportunities to work more effectively.

Within ACC and the Honouring Life Education framework, it helps bridge gaps between services, reduce siloed thinking and create more connected support through life's transitions.

Joint activities, workshop leading, education and keynote speaking opportunities are part of the collaboration arm we are developing.

Contribution recognises that everyone has something valuable to offer, whether that's specialist knowledge, lived experience, a useful resource or simply a fresh perspective.

By sharing what we know and learning from others, we strengthen the ACC community and support HLE's vision of a more informed, empowered and compassionate community.

Sharing events, opportunities, invitations, training programs, resources, and services helps everyone learn and access what may not previously have been on their radar.

The ripple effect is considerable as more people within the professional community become more aware, and some may go onto upskill, benefiting many more.

Professional Hub,

Bringing the experts to the community.

Purpose: To help reduce the barrier of uncertainty- not knowing what they don't know. So attendees can ask specific questions and explore multiple ideas and interlinking topics at the same event.

Similar to the professional Meet and Greet, except we, the professionals, initiate, host, or attend community events.

This can be through

workshops,

seminars,

speaking,

expo information stands,

online webinars,

conversations,

Q&A sessions, etc.

The options are almost limitless, and as a collective, the potential is significant.

The format is open and matched to external requests, what we collectively decide to offer, or your own collaboration.

We believe aging and dying are very much about living well. We create opportunities for families, carers, professionals and individuals to connect to stay well.

We support this by bringing people, professions, businesses, and organisations together so they can discover who is doing what, understand each other's work, and build meaningful relationships.

Knowledge is Empowerment: Contribute

Build awareness, understanding and confidence so people can make informed decisions, recognise their options and navigate life's transitions.

This is achieved within the professional layer through ACC and spread into the community via providing information, events, connections, and support in various ways to help others regain a sense of

control when they feel powerless,

confidence when overwhelmed

and clarity when confused.

Whole-Person Care:

We value individuals and their support (carer) teams’ emotional, physical, spiritual, mental, and practical needs at each stage of the journey.

Inviting professionals and businesses across the breadth and depth of the individual's (client, carer, and family) living and care network enables a closer network, working to minimise cracks and strengthen pathways for optimal living, care, and thriving.

Gentle Guidance:

Encourage the exchange of knowledge, ideas, and resources, opening opportunities to bridge gaps, work across professional boundaries, and strengthen support.

There are many ways to achieve this.

Professionally, this includes reflection, building networks, and having meaningful conversations during the Meet and Greets, sharing stories, and offering workshops and resources.

Inclusive and Respectful Collaboration

We appreciate that how people navigate and feel about aging, death, dying, and grief is personal and is different across cultures, beliefs, and experiences.

Everyone is invited to share their lived experience, culture, heritage, and perspectives, which will enrich others' insights.

This insight can increase empathy, compassion, and curiosity, and improve support through expanded awareness, consideration, and sharing useful connections and information within their own networks. This is where individual knowledge creates a wider community ripple.

Where it began — and where it can lead

Honouring Life Education (HLE) was born from understanding what it feels like to be overwhelmed by information, go down too many rabbit holes and still struggle to find what you need.

After hearing similar stories from others, the idea was simple: bring useful information, resources and support together in one place, supported by education and events that help people make sense of it all.

But as conversations with people across the care sector grew, another gap became clear.

So many excellent professionals, businesses, organisations, resources, and ideas are doing valuable work. Yet, no matter how experienced we are, we can't know about them all — particularly those working beyond our own professions and networks.

What if we could change that?

That question helped shape Aligned Care & Connect (ACC): a space to bring people and services together, build understanding of what each other offers, share knowledge and discover opportunities to work together.

Because sometimes, one conversation is all it takes for the lightbulb to come on. A new connection, a shared idea or a piece of information might be exactly what someone needs — whether that's a client, colleague, friend, parent or neighbour.

And that's where the ripple begins.

When we share what we learn with our own networks, the knowledge travels further. One connection leads to another, new possibilities emerge, and the benefits can reach people and communities well beyond the original conversation.

That's the bigger picture behind Honouring Life Education and Aligned Care and Connect: bringing people, knowledge and resources closer together so we can learn from one another, support one another and make a difference together.
